Now that you have chosen CFR as your HHCAHPS vendor, we can get started collecting your data. To get started, you will need to do the following things:
- Fill out the HHCAHPS consent form (available at https://homehealthcahps.org/) and log in and update your passwords.
- On that website, HHAs will need to authorize CFR as their vendor to allow us to submit data on your behalf.
- Provide CFR with your CMS Certification Number (CCN, previously known as Medicare Provider ID Number) and legal Name of the HHA to be used.
- Patient files (information about patients) must be sent to CFR every month. We need to receive it on or before the 10th of each month.
Information, on all of the Medicare/Medicaid patients that received service during the calendar month, should be sent to CFR and we will sort through who is eligible to call. When sending patient information, you may send it using a password protected and/or encrypted features to ensure confidentiality. CFR will complete the process, taking care of everything else for you - including creating a list of patients that are eligible, creating a list of patients to contact, contacting the patients, collecting the data and sending your data to CMS.
