Now that you have chosen CFR as your HCAHPS vendor, we can get started collecting your data. To get started, you will need to do the following things:
- Sign up with QualityNet.org to allow us to submit your hospital’s data. CFR can provide this form to your hospital with our company information already filled out; your hospital will just need to sign and mail in.
- Provide CFR with your hospital’s MPN (also called CMS certification number or CCN) and legal name of the hospital to be used with the records.
- Provide CFR with the name and number of a person whom patients with questions about the legitimacy of the survey can contact at the hospital.
- Your patient file (information for discharges) must be sent every month. This can be sent once a week, once every two weeks or once a month, but we need to receive it on or before the 5th of the month following the discharges (for example, patients discharged in January must be sent by the 5th of February).
Data from all of the patient discharges should be sent to CFR to sort through eligibility. When sending patient information, hospitals may send it using a password protected and/or encrypted features to ensure confidentiality. CFR will complete the process, taking care of everything else for you - including creating a list of patients that are eligible, creating a list of patients to contact, contacting the patients, collecting the data and sending your data to CMS.
